That accessibility is genuinely valuable—but it has also created a new problem: data without context.

Ordering a testosterone test is the easy part. Knowing which test to order, when to collect the sample, what supporting markers to include, and how to interpret the results in a meaningful way—that is where most men get lost. This guide is designed to provide that framework.

The Foundation: Why One Number Is Never Enough

The phrase "testosterone level" is deceptively simple. In clinical practice, testosterone circulates in the blood in several distinct forms, and each tells a different part of the story.

Total Testosterone measures the combined concentration of all testosterone in the bloodstream—both the portion bound to proteins and the portion that is biologically active. It is the most commonly ordered test and serves as a useful starting point, but it does not tell you how much testosterone is actually available to your tissues.

Free Testosterone represents the unbound fraction—typically just 1 to 3 percent of total testosterone—that circulates freely and can enter cells to exert its effects. This is the biologically active portion. A man with seemingly adequate total testosterone but low free testosterone may still experience symptoms of deficiency, because the active hormone is simply not reaching its targets in sufficient quantities.

Bioavailable Testosterone captures both free testosterone and the portion loosely bound to albumin (a blood protein). Unlike the tightly bound fraction attached to sex hormone-binding globulin (SHBG), albumin-bound testosterone is readily released and biologically accessible. Some clinicians consider bioavailable testosterone a more complete picture of androgenic activity than free testosterone alone.

When establishing a meaningful hormonal baseline, ordering all three measurements—or at minimum total and free testosterone—provides substantially more actionable information than a single figure.

Understanding SHBG: The Protein That Governs Availability

Sex hormone-binding globulin is a protein produced primarily in the liver that binds tightly to testosterone, rendering it biologically inactive. SHBG levels have a direct and powerful influence on how much free and bioavailable testosterone a man has access to, regardless of what his total testosterone reads.

Elevated SHBG is common in older men, men with hyperthyroidism, and those with high-fiber or very low-fat diets. When SHBG is high, a large proportion of total testosterone is effectively locked away—which can produce symptoms of low testosterone even when total levels appear normal on paper.

Conversely, low SHBG—often seen in men with obesity, insulin resistance, or hypothyroidism—can mean that more testosterone is freely circulating, but this pattern also carries its own metabolic implications.

Testing SHBG alongside testosterone is not optional for men who want a complete picture. It is essential.

Estradiol: The Hormone Men Overlook at Their Peril

Estradiol (E2) is the primary estrogen in men, produced through the conversion of testosterone by the aromatase enzyme. Maintaining estradiol within an appropriate range is critical for bone density, cardiovascular health, libido, mood regulation, and joint comfort.

Many men are surprised to learn that both too little and too much estradiol can impair wellbeing. Low estradiol—sometimes seen in men on aromatase inhibitors or those with very low body fat—is associated with joint pain, low libido, and mood disturbance. Elevated estradiol, which can accompany high testosterone levels or increased aromatase activity in adipose tissue, may produce water retention, emotional volatility, and reduced sexual function.

For men undergoing hormone optimization protocols, tracking the testosterone-to-estradiol ratio over time is a standard component of responsible monitoring. For men conducting a general baseline assessment, including a sensitive estradiol assay (specifically the LC-MS/MS method, not the standard immunoassay) provides meaningful additional context.

Timing Your Test: When You Draw Blood Matters

Testosterone is not a static value—it fluctuates significantly throughout the day, peaking in the early morning hours and declining through the afternoon and evening. Research has documented differences of 20 to 35 percent between morning and afternoon measurements in the same individual on the same day.
